I, hereby grant consent to LaToya Duncan Photography, Mika Flur Photography, and Traditional Centers to use my likeness, including photographs, audio recordings, and video recordings, as part of the photography project titled "Celebrating Brooklyn Mothers". I understand and agree to the following terms:Purpose of Use
I understand that my likeness will be used for the purposes of the Project, which aims to showcase and celebrate the experiences of Brooklyn mothers, as described in the project description.Use of Likeness
I grant LaToya Duncan & Tamika Faurelus, and Traditional Centers the right to use, reproduce, publish, distribute, and display my likeness, including but not limited to photographs, audio recordings, and video recordings, in connection with this Project. This includes both digital and print formats, as well as any promotional materials related to the Project.Release and Waiver
I hereby release LaToya Duncan & Tamika Faurelus, and Traditional Centers from any and all claims, demands, or liabilities arising out of or in connection with the use of my likeness as described in this consent and release form. I understand that once my likeness is published, it may be viewed by the general public and cannot be controlled or protected from unauthorized use.Confidentiality
